Elaya Health is here to relieve the burden on nursing homes and long term care facilities by providing their residents with meticulous diabetes management.
According to the CDC, 37.3 million Americans—about 1 in 10—have diabetes. And about 1 in 5 people with diabetes don’t know they have it. We are here to educate and treat the most vulnerable population in the nursing homes and LTC facilities.

DR. MEHRI SONGHORIAN
Doctor
Dr. Mehri Songhorian is a Neurologist in Elmhurst, New York and Great Neck, New York. She is affiliated with multiple hospitals in the area, including Long Island Jewish Medical Center and New York Hospital Queens. She received her medical degree from Tehran University of Medical Sciences and has been in practice for more than 20 years. She is one of 45 doctors at Long Island Jewish Medical Center and one of 22 at New York Hospital Queens who specialize in practicing Neurology.
DR. DON ZWICKLER
Doctor
Dr. Don Zwickler, MD is a board certified endocrinology, diabetes & metabolism specialist in Pomona, NY and has over 38 years of experience in the medical field. He graduated from New York University, School of Medicine in 1984 and completed his residency at Brookdale Hospital in 1987. He is affiliated with Good Samaritan Hospital and is part of a group practice Endocrine Associates of Rockland County.
